At ELIH, the goal of educating the patient and family is to improve patient health outcomes by promoting recovery, speeding return to function, promoting healthy behavior and appropriately involving the patient and family in their care and care decisions.

TAKE CHARGE OF YOUR CARE

SPEAK UP. Ask questions and voice concerns. It’s your body and you have the right to know.

PAY ATTENTION. Always double-check that you are getting the right treatments and medicines.

EDUCATE YOURSELF. Learn about your medical condition, tests and treatment options, so you know why following your care plan is so important.

FIND A SUPPORT PERSON. Pick someone to help speak up for your care and needs during your stay.

KNOW YOUR MEDS. Understand what your medicines treat, why you need them and how to take them for the best results.

PARTICIPATE IN YOUR CARE. You are the center of your healthcare team. Know what is happening every step of the way— from admission through discharge. Ask your nurse about the patient information portal. It allows you to access your inpatient medical record when you return home. To learn more, call 631-477-5711.

Senior Wellness Education Series – This free education series features timely topics specifically targeted for older adults, their family, friends and caregivers. Lectures are held on the first Friday of every month from 11 am – 12 noon at Southold Town Senior Services, 750 Pacific St., Mattituck.
